It’s called Customised Forms – a nifty new Shortcuts tool that enables you to collect consent and other important information from your clients before their appointment. How do they work?
Customised Forms are super simple to set up thanks to a handy what-you-see-is-what-you-get layout that lets you see your form as you build it. You’re given plenty of client information fields to choose from, so you can gather the key info you need, from allergies, to high blood pressure, to details about their upcoming appointment. Nothing will slip past you!
Simply select the fields you’d like to include, and customise them with the questions you’d like to ask. It’s a cinch. You can also choose how often you’d like each client to fill out the form – this can be just once, every visit, or every few months. The power is in your hands.
Once the form has been created and saved, simply hand a tablet to your client so they can fill it out. They can add their signature at the end of the form, and once they agree to the terms and conditions a time stamp will appear, meaning you now have formal signed consent for their procedure.
The form will be saved nice and safe in their online client history which you can view as necessary. Say adios to tedious admin work and data entry mishaps.
